Proof of identity

Recommended: the company must verify your identity. Enclosing a copy avoids queries and delays. Black out what is not needed (e.g. document number, height) – photo, name and date of birth remain visible. Write across the copy: “For identification only – request of [date]”.

Envelope

The address is printed so that it appears in the window of a C5 envelope. Print at actual size (100 %, not “Fit to page”) and fold the A4 sheet once in the middle.
