Recommended: the company must verify your identity. Enclosing a copy avoids queries and delays. Black out what is not needed (e.g. document number, height) – photo, name and date of birth remain visible. Write across the copy: “For identification only – request of [date]”.

Request for access under Art. 25 FADP

Dear Sir or Madam,

Pursuant to Art. 25 FADP, I request that you inform me whether you process personal data relating to me.

If so, please provide me in particular with the following information (Art. 25 para. 2 FADP):

your identity and contact details as controller;
the personal data processed as such;
the purpose of the processing;
the retention period or, if this is not possible, the criteria used to determine it;
the available information on the source of the personal data;
where applicable, the existence of automated individual decision-making and the logic on which it is based;
where applicable, the recipients or categories of recipients and, in the case of disclosure abroad, the country and the safeguards (Art. 19 para. 4 FADP).

Please also send me a copy of my personal data in a commonly used electronic format to the following email address: [your email address] (Art. 25 FADP).

The information must be provided free of charge (Art. 25 para. 6 FADP) and within 30 days of receipt of my request (Art. 25 para. 7 FADP and Art. 18 para. 1 DPO).

To prove my identity, I enclose a copy of my identity document. It is to be used solely to identify me; please delete it once this has been done.
